Using FromPlugin to create a form, which sends an email with attachment on SUBMIT

Hello,

i want to use the FormPlugin to create a from, which implements the following functionality:

1. Beside several text-fields, a wiki-user should also be able to add an attachment-file to the form.

2. After a click on SUBMIT, all the data (including the attachment) should be sent to a fixed email-address of our company.

As i am no perl nor an html or javascript expert, it would be very helpful for me, to get some starting hints, how
to use the FormPlugin to achieve the functionality described above. Is it nececessary to write an own perl-script (e.g. "sendemailxyz.pl")
for the "action" field, or is there a way to use intrinsic features of FOSWIKI to implement item 2.?
